Note about Out-of-Network Services

The plans will pay covered out-of-network charges up to what is considered a “reasonable and customary” (R&C) rate. This rate will be the lesser of the amount charged or 150% of Medicare allowed charges.

If you are charged an amount that exceeds the R&C limit, you must pay the difference. Please contact Blue Cross Blue Shield if you have any questions.
